GW Hoops liability waiver

Assumption of Risk and Waiver of Liability: I, the undersigned participant, and if under 18, my parent or legal guardian, acknowledge that I am voluntarily participating in the GoodWin Hoops basketball program. I understand and acknowledge that basketball involves physical activity and inherent risks. I am aware that these risks may include, but are not limited to, sprains, strains, fractures, and other injuries, including the potential for serious injury, and I accept those risks.

Release and Waiver: In consideration of being allowed to participate in the GoodWin Hoops basketball program, I, on my behalf, and on the behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless, GoodWin Hoops, its directors, officers, employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in the program or any related activities, or while on the premise where the program is being conducted.

Medical Treatment Consent: I hereby consent to receive medical treatment that may be deemed necessary by medical professionals in the event of injury, accident, or illness during my participation in the GoodWin Hoops basketball program.

Phots/Video Release: I grant permission for GoodWin Hoops to use photographs and videos taken during the program for promotional and marketing purposes.

I have read this waiver and fully understand its contents. I am aware that by signing this document, I am giving up substantial rights, including the rights to sue.
